Directions to Register my parent to play sports at Strong Middle School.
 
In order to participate in any interscholastic sport at Strong, the following requirements must be met prior to your child beginning try-outs or practices for each individual sport:
 
Important - Please note that in order to begin any school based sport, you MUST be registered under our new registration program, FinalForms.  Please turn in any updated physical examination at your earliest convenience, however you will NOT be permitted to begin that said sport’s practice without a valid physical examination on file.  All Physical Medical Examinations MUST be submitted to the CRHS Nurse’s office in hard copy form and can be dropped off in the main office.  Please make sure this is accomplished 3 weekdays PRIOR to the start of tryouts.  
 
All of the following sports are open for ALL 6th, 7th, and 8th grade students to try out for, however depending on the number of registrants, we may decide to split tryout dates by grades or another method.  Unfortauntely, students in Grade 5 are NOT permitted to try out for Middle School sports. Please note - All updated physicals should be dropped off to Strong Middle School Nurse in the mailbox of our School Nurse (Hardcopy ONLY) prior to tryouts beginning.
 
We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).
 
1. Completion of FinalForms Sport Registration for Interscholastic Athletics.  The active registration link is the same for for each season, but you will not be able to register for other seasons unless the below dates.
  • Fall Sport Registration is NOW OPEN open on Friday July 31, 2026
  • Winter Sport Registration will open up on / about October 30, 2026
  • Spring Sport Registration will open up on / about January 30, 2027
 
2. Unified Sports FinalForms registration link - Active entire year 2026-2027.
  • Link will be posted closer to th estart of the 2026-2027 School year
  • Parents, please remember that ALL Unified Sports participants will require an updated physical examination within the past 13 months.  If you have had one updated, please remember to drop off a hard copy to the school Nurse at your earliest convenience.  
  • Unified Sports activities will not begin until after the first day of school.  If you have any questions regarding Unified, please refer to our Unified Sports page for more information.
 
 
3. A Sports Health Assessment Record (Physical) completed by the Physician and Parent/Guardian must be submitted directly to the Strong school nurse, Anne Juncadella . In order to be considered valid, the "normal" date of the physical examination can be no more than 13 months old. You must submit a copy of your latest health physical if it is not current.
